What is Alfi Telehealth? 

Telehealth is an easy-to-use, home monitoring system, individually tailored to support patients living with long term conditions. Once installed in the patient’s home, Telehealth equipment keeps a regular check on vital signs such as blood pressure, blood sugar levels, oxygen levels, and weight, and sets up an ‘early warning system’ to detect changes.

Using a home-based technology is an effective way of supporting patients to remain in their own home for longer and supports the management of long-term conditions in our rural communities. 

The service is delivered from within Cornwall by a team of friendly, experienced health professionals.

Alfi gives you the right equipment to take key health readings yourself, which takes just a few minutes a day. Using your normal phone line, these readings are automatically sent to a health professional – usually your key worker (Community Matron or Specialist Nurse). This means that they can quickly spot any change in your condition and follow up on this if necessary.

The best thing is that there’s expert help at the other end of the line if and when you need it. So you have the confidence to get on and enjoy your life!

Who can benefit from Alfi Telehealth?

Our Telehealth Service is now supporting over 950 patients across Cornwall and North Devon. We closely monitor patient readings, looking for trends and possible issues, and contact patients to offer support.

We have found that patients using Telehealth learn more about their condition, which encourages independence and quality of life.

Alfi can help people with a range of long-term conditions, including:

  •          Cardiovascular (heart) disease
  •          Chronic obstructive pulmonary disease (lung disease)
  •          Diabetes
  •          Hypertension (High blood pressure)
  •          Urinary tract infections

It can help people manage their conditions and reduce the risk of:

  •          Falls
  •          Stroke
  •          Heart failure

How does it work?

Alfi’s equipment is really simple to use, for people of all ages. You don’t need to know anything about computers!

We’ll visit your home to set up the system. The exact equipment will depend on your condition, but none of it is complicated and it will take up very little room. We’ll show you how to use it, and we’re always ready to help if you have any questions.

At set times each day, you use the equipment to take readings of your ‘vital signs’ such as blood pressure, blood glucose and pulse oximetry (an easy way to measure the oxygen in your blood).

No need to worry about remembering when to take your readings – Alfi will remind you. Your key worker may also set the system to ask you some simple questions about your health, depending on your condition.

Once you’ve taken your readings, this information is sent automatically via your phone line to Alfi’s Telehealth Nurse Team. A ‘traffic light’ system helps them spot any changes in your condition that might need attention – and also helps you to monitor and understand your own condition.

If your readings do show a problem, one of our team will get in touch with you to make sure you’re OK and advise you on what to do.

How can I start using Alfi Telehealth?

Your key worker (Community Matron or Specialist Nurse) or your GP can make a referral if they feel you could benefit from Alfi Telehealth.

One of our team will then visit your home to give you more information about the equipment and how it works. If you decide to go ahead, we can install the equipment immediately.

We’ll show you how to take your readings and answer any questions you may have. And we’re always there to help if you need any further support in using Alfi Telehealth.
