Cardiac Services

Team

We offer a county wide service. There are four distinct teams based across the county who offer cardiac care and support. The teams consist of specialist cardiac nurses, physiotherapists, physiotherapy support workers and an exercise specialist.

Our teams work closely with colleagues in acute hospitals to ensure the most effective delivery of services to patients within the community.

Cardiac rehabilitation gives the individual, families and / or carers the information, support and advice needed to recover and maintain heart health.

 Accessing the Service

Cardiac rehabilitation is delivered in a variety of settings; including at home, in a clinic setting and/or as a group programme, depending on an individual’s needs.

Our aims:

  • To deliver a comprehensive cardiac rehabilitation programme to all patients who have experienced an acute cardiac event, this includes post myocardial infarction (heart attack), cardiac surgery (by-pass and / or valves), the insertion of an internal cardiac defibrillator, coronary angioplasty or heart failure.

Cardiac rehabilitation gives the individual, families and / or carers the information, support and advice needed to recover and maintain hearthealth. There is a wealth of evidence to support cardiac rehabilitation with regard to improving outcomes for many people with heart disease enabling them to remain active longer and to manage their own condition more effectively.Cardiac rehabilitation is delivered in a variety of settings, including within the home, a clinic setting  and/or as a group programme, depending on an individual’s needs.

Cardiac rehabilitation includes:

  • A comprehensive nursing / physiotherapy assessment
  • The delivery of a programme that meets the needs of the individual and includes the following core components, relevant lifestyle advise, risk factor management, review of cardio-protective drug therapy, psychosocial and quality of life issues, education, exercise and an understanding of long term management
  •  The service also offers individual support and care to the more complex / compromised cardiac patients within the community. This service is predominantly aimed at individuals with proven heart failure

Individuals with heart failure are prone to periods of instability and frequent hospital admissions. The Cardiac specialist nurses support individuals in order to help prevent unnecessary hospital admissions and  to improve quality of life.

The nurse’s work closely with general practitioners and consultant cardiologists to ensure an individuals medications are optimised in order to improve symptoms and prognosis.

The nurses also support end of life care when required.

Service availability

The service is available Mon – Fri 09.00 hrs to 17.00 hrs (excluding Bank Holidays).
