Information Governance

Is a term used to describe how we manage information legally, securely, and effectively, in order to deliver the best possible care.

Because of the range and complexity of the standards and legal rules, the Department of Health developed standards to allow NHS and Partner organisations to measure their compliance via the Information Governance Toolkit.

Requests for Information

Personal Information

The Data Protection Act 1998 gives living individuals the right to request access to personal records held by Peninsula Community Health, this is called a Subject Access Request (SAR). Please find our application form below.

The Access to Health Records Act 1990 gives access to the records held on the deceased created since November 1991. Personal representatives, administrators and executors or an individual with a claim resulting from the death can make an application using the Health Record Application form below.

If the request is to access acute records including:

  • X-Ray please refer to Royal Cornwall Hospitals Trust
  • Mental Health and Childrens Services refer to Cornwall Partnership Foundation Trust
  • Continuing Care refer to Kernow Clinical Commissiong Group
  • GP records directly to the GP practice manager.

The Freedom of Information Act grants a right of access to ‘information’ held by public authorities.  As Peninsula Community Health is not a public authority it is not subject to the FOIA, however as an organisation we are comitted to being open and transparent in our communcations and will provide accurate, up to date information on request. How we manage your information

We are migrating to an electronic health record system (SystmOne) to record the contact and appointments we have with patients. For more information on how we are using SystmOne and frequently asked questions, visit our Electronic Patient Records page.

In addition to the electronic records we will also retain historical paper records as well.
