The Adult Speech and Language Service provides diagnosis and treatment for adults with communication and swallowing difficulties.
Health professionals may click to download the Referral form and send to Referrals.AdultSLT@pch-cic.nhs.uk Alternatively a letter may be sent to the same referrals site. You can ask your GP or other health professional to make a referral on your behalf. They can use our referral form, email us or send a letter to Referrals.AdultSLT@pch-cic.nhs.uk
Swallowing problems (Dysphagia) can occur for many reasons. They are sometimes experienced after a stroke, dementia, Parkinsons, MS or as a result of stress.
The Adult Speech and Language Service sees people 18 years and above with a range of communication and swallowing difficulties.
Aphasia is a condition, which affects a person’s ability to communicate. This can include difficulties with talking, finding the right words and understanding what people are saying.
Dysarthria is a speech difficulty caused by paralysis, weakness or poor co-ordination of the muscles we use for talking.
Voice problems (Dysphonia) can occur for many different reasons. If you have a voice problem your doctor will send you to an Ear Nose and Throat Department (ENT) where your vocal folds will be looked at. You may then be referred to a SLT voice therapy information session. If you require individual voice therapy an appointment will be arranged with a SLT/Voice Therapist who will take a detailed history of your voice problem and devise a Voice Therapy plan specifically for you.
Dyspraxia is a disorder of speech which is caused by damage to parts of the brain related to speaking e.g. following a stroke or traumatic brain injury. People with dyspraxia know what they want to say but have difficulty coordinating the muscle movements needed to say the words
Stammering or stuttering is where the smooth flow of speech is interrupted. How this happens is very individual to the person with the stammer. Not only does stammering vary from individual to individual but the intensity of the stammer can vary throughout the day.
The Adult Speech and Language Therapy service sees many people who have difficulties in speech, communication and/or swallowing arising from Parkinson’s Disease.
Communication Aids are used to assist or replace speech. These can include writing, drawing, gesturing, signing and using a range of devices
There are many residents in Nursing Homes who may experience communication or swallowing difficulties. In order to access the appropriate service to assist with their problems, they rely on the skill of the Nursing Home staff to trigger an appropriate referral.
The Adult Speech and Language Therapy service co-ordinates its own successful volunteer scheme. We are always looking for volunteers to assist our patients in their home environment.
The Speech and Language Therapy service can provide a range of training sessions for organisations, groups or individuals on any topic related to speech, voice, communication or swallowing.

Everyone has the right to be continent, whenever this is achievable. A person has the right to the highest standards of available health care and social care to ensure an optimum quality of life, independence and personal dignity.
The pelvic floor muscles form a broad sling at the base of the pelvis. They help support your bladder, rectum (back passage) and in women your uterus (womb). They prevent you leaking urine or faeces.
In overactive bladder, this ability to override the bladder messages is reduced. This results in more frequent visits to the toilet, often quite urgently and sometimes not getting there in time.
This diary helps you and us to understand why you get trouble with your bladder. The diary is a very important part of the tests we do, so that we can try to improve your symptoms. Please choose any three days (these do not have to be consecutive days) between receipt of the diaries and when you attend your appointment with the Continence Promotion Service.

Welcome to the Tuberculosis Service. We are a small team of specialist nurses covering the whole of Cornwall and Isles of Scilly. We provide support to people diagnosed with tuberculosis and also run contact tracing and vaccination clinics.
What is Tuberculosis? Tuberculosis (TB) infects both humans and animals. In the UK, the most common site of infection in humans is the lungs, although any part of the body can be affected.
What is Bovine Tuberculosis? Tuberculosis (TB) is caused by germs/bacteria known as mycobacterium (M.). The germ known as M. Bovis can cause TB in cattle called Bovine TB. In Cornwall bovine TB has also been found in pets and other livestock.

Treatment Available When you are diagnosed with tuberculosis, you should be referred to us and we will provide support during your treatment and recovery.
Treatment and Support for Patients Diagnosed with Tuberculosis The standard treatment for Tuberculosis (TB) is a six month antibiotic course, although some forms of the disease may require longer treatment. Your Consultant will decide on the length of treatment.
Treatment for Your Family and Friends When you are diagnosed with Tuberculosis (TB) you will be assigned a named TB Specialist Nurse. We will not only support you but also your family. Family members and close friends have a slightly higher risk of developing TB.
Vaccination Clinics Vaccination against Tuberculosis is a two stage process. Firstly, we will administer a Mantoux skin test in accordance with NICE guidelines. The results of this test need to be read 48 hours later and given a BCG vaccination if required.
Eligibility BCG Vaccination is given to provide protection against Tuberculosis (TB) but it is no longer given as part of the routine NHS childhood vaccination schedule. Each child is assessed individually and, if increased risk is identified, will be vaccinated at the earliest opportunity, e.g. at birth.
How do We Identify Children for Vaccination? Children are no longer vaccinated as part of a routine vaccination program. Each child is assessed individually and, if increased risk to Tuberculosis (TB) is identified, will be vaccinated at the earliest opportunity.
Private Patients For those who wish to receive BCG vaccination but are not eligible on the NHS, we offer a small number of private patient appointments.

Mantoux Skin Test Information A mantoux skin test checks the immune system’s response to Tuberculosis (TB) bacteria. It is a small injection into the top layers of the skin on the left forearm.
How is the BCG Vaccination Given? The nurse will give the vaccination by injection into the left upper arm. The needle will go into the top layers of skin.
How to Care for the BCG Injection site The site of the injection (upper left arm) will be sore for a few days but it is unlikely to make you feel unwell or feverish.

Knowing the person is key to understanding the needs of a patient. There is an absolute need to embed person centred care within all organisations and we are working hard to make our staff and environments are dementia friendly.
Our staff are passionate to ensuring patients with dementia receive the best care possible. This involves getting to know the patient and working with them to build a one page profile clearly outlining their likes, dislikes and preferences.
We have two interactive software devices to aid patients, carers and nurses to speak with patients. The screen can be wheeled to the patient’s bedside where they can listen to music, digitally paint and play games using different tools available.
There’s a lot you can do to maintain your own memory, and memory cafés are a great way to meet others, socialise, take part in fun activities and re-build confidence.
